About Takako

Takako Shiojima, RD, has been practicing as a registered dietitian since 2008, with experience across gestational clinics, short-term acute care, long-term acute care, PACE programs, and nursing homes. She also served for seven years as Director of the Nutrition Department at a long-term acute care facility.

Approach to care

Her approach to care is patient-centered and collaborative, grounded in the belief that sustainable health changes begin with understanding each individual’s unique medical history, lifestyle, culture, and personal goals. With extensive experience across gestational clinics, short-term acute care, long-term acute care, PACE programs, and nursing home settings, she brings both clinical depth and practical insight to every client interaction.

Your first session with Takako

Care begins with listening. During the initial session, she focuses on building rapport and creating a supportive environment where clients feel heard and respected. A comprehensive nutrition assessment is conducted, including a 24-hour dietary recall to better understand current eating patterns, food preferences, and daily routines. This assessment is not about judgment, but about identifying strengths, challenges, and opportunities for improvement. Medical history, lab values (when available), medications, and symptoms are carefully reviewed to ensure nutrition recommendations align with overall health needs.
